Entry Forms are available from Club Handicapper after each Club Qualifying Tournament.

Players with a H.I. of 18.5-36.4 must play to a H.I. of 18.4 except for Tournament #4.

Entrants must complete their own "Official Entry Form" (including fees and signed verification by the Club Handicapper) and transmit it prior to the entry deadline.
